- IdentifyUS Specimen Evaluation Form
Specimen info and images aebMergere submitted to an expert entomologist at IdentifyUS via a Formstack form. Customer receives back a formatted PDF doc with images and evaluation results that they can share with their physician. - IdentifyUS Tick Pathogen Testing
Customers can elect and pay for various tick pathogen tests via a Formstack form. Conditional logic and payment gateway interface produce a PDF formatted summary used by clinician to followup with patient. Copies are emailed to customer and placed in cloud storage repository automaticaly. A copy is also sent to the Xero accounting system to be attached as receipt on an AP invoice. - Oyster Conservation Field Data Entry & Reporting
Prototype system developed for The Nature Conservancy oyster conservation project to record oyster spat measurements and cage data in the field. Coordinators gain access to a spreadsheet filled with volunteer entereed data without having to transcribe it again themselves. - TNC Liability Waiver form
A sample liability waiver form for The Nature Conservancy oyster conservationist volunteers. Returns a formatted PDF doc to all parties and files it on cloud storage. - Tuckerman Inferno Race Registration
Uses conditional logic to register racers for a multi-stage Pentathlon endurance event for the Friends of Tuckerman Ravine. Racers receive back a formatted team registration form and liability waivers associated with each team member. Race officials gain access to a spreadsheet of registered racers as they sign up. - Gift In Kind Donation form
Provides an automated mechanism for vendors to record their donation and submit company logo and gift documenation at the same time. Generates a receipt that can be used as documentation for tax purposes. - Nonprofit Conflict of Interest report
Nonprofit board members can automatically sign and submit annual conflict of interest reports online and those signed documents are routed to wherever they need to land.
